Healthy Hearts Initiative

How the program works:

Participants will be required to measure their blood pressure at least two times a month for four consecutive months - a minimum of just eight times.  They will upload their blood pressure readings using Heart360, an easy-to-use webiste that enables particiants to record blood pressure and other data such as weight, blood glucose and more.  Heart360 delivers weekly emails encouraging participants to monitor their blood pressure.

Participants will work with a Healthy Heart Ambassador appointed by the Y.  The Ambassadors will show participants how to use a self-monitoring blood pressure cuff and how to record blood pressure readings using the Heart360 website.

The Healthy Heart Ambassadors will periodically check in with participants to encourage them to monitor and record their blood pressure.  They will also hold regular office hours to assist participants.

Monitoring Matters - Y-USA is one of several partners in the Million Hearts campaign launched by the U.S. Department of Health and Human Services.  The goal of the campaign is to prevent one million heart attacks and strokes by 2017 through heart disease prevention activities.  The Ann Arbor YMCA is one of three Ys participating in a pilot program to help prevent heart disease through self-monitoring and recording.
